I, as a participant or parent/guardian of the participant in the programs sponsored by the Boulder City Parks and Recreation Department, recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, including death, damages, or loss regardless of severity which I or my child may sustain as a result of participating in the programs. I fully release and discharge the City, its officers, agents or employees from all liability demands or claims for loss, damage, death or injury resulting from participating in the programs. I have been informed by the City and I fully understand and agree that no insurance coverage whatever is being provided by the City, its agents or representatives, undertaking to indemnify, or in any other manner, guaranteeing the safety of any participant in the programs as to any injury or other peril or contingency resulting from participating in the programs, whether to persons or property. I recognize and understand that the programs requires that the participant be in good health. I warrant and declare that the participant is in good health. If the participant is a minor, I also give my consent for his/her articipation in the programs, and for any necessary emergency medical treatment.

PHOTO RELEASE: I, the parent/guardian, hereby consent to & authorize the use and reproduction by Boulder City Recreation Department of any photographs & audio-visual materials taken for marketing materials or for any other use for the benefit of the program. When any media of participants do appear in marketing materials, on the website, or social media there will not be any personal identification (i.e., name).
